
Family Member Sign-Up Step By Step
- To become a member of the Edmonton Epilepsy Association, complete the form, below, please enter in your information with the following format:
Name: Enter your FULL NAME (First and Last Name)Contact Person: You can enter your full name again.
Address: Enter your street address (eg: 1234 – 45 Ave SW) in the first box. In the following box enter your City, Province, Postal Code (please use a comma to separate them) . Please enter the separate City, Province, Postal Code as well.
Email: enter your Email address (this is where we’ll send your sign in info and receipt)
Phone: enter your phone number. (The phone your normally use) - Next, select your membership term. You can choose Annual or 3 Year. The 3 Year term is discounted.
- After choosing your membership term, choose your payment method. For 1 year memberships, you can choose to use your credit card to pay for the upcoming year and then it will be used to automatically renew the next year OR you can pay for the upcoming year by credit card and then manually renew next year after OR you can pay by cheque or Interac payment for the upcoming year and then manually renew next year after (payments need to be received by the EEA within 15 days}.
- Credit Card Payment: Fill in the Name from your card (this should match the name on your card), Billing address and city, province, postal code then your credit card number with expiration date and the 3 digit CVC code on the back.
- If you have been provided a special discount code, enter that code exactly as provided in the Discount code box and click apply.
- Then click the Sign Up! button. Your payment will be processed, and you will be automatically signed into the Member Account Management page. An Email will be sent to you with your sign in information and your account payment receipt or instructions.
- After successfully completing the payment, you’ll be taken to the account management profile where you can provide us with some additional information and confirm your communications preferences.
- Click the FamilyMembers tab so you can enter information about each family member. You will add them as a contact. You only need to enter their first name, how they are related to you (son, daughter, wife, husband, etc.). It is not necessary for you to enter their address as it would be the same as yours. You can optionally enter an Email address for them (it must be different from the one you used to sign up). This Email address will be used for them to sign into their members area of the website. You do not need to enter Phone, Mobile or Fax. Under additional info, please enter the year of birth if the child is under 18. This helps us with planning events and services for children. Click SAVE. You can add up to 4 additional family members. When you’re done, click Save & Continue.
- Next, you’ll go to page to confirm your account management page. After you review the information on each Tab, please click Save & Continue. If you need to make any changes, please do so and then click Save & Continue.

After you’re done, you’re briefly stop at a completed page and then you’ll be redirected to the Edmonton Epilepsy home page.
